Worksheet · moraph.org/commons/sessions/reach/finding-low-cost-care
Use these pages during the session and keep them. Nobody has to share their answers, and the clinic cards are made up.
1 The call script
Practice saying this out loud: “Hello, I’d like to ask about seeing someone. Are you taking new patients? What does a visit cost, and do you have a sliding scale or a reduced fee? Is there anything I need to be seen? What should I bring? Can I have an interpreter?” Write any words you would change.
2 Made-up clinic cards
Clinic A: taking new patients; full fee 100, sliding scale from 20 with proof of income; bring ID if you have it and a recent pay slip; interpreter by phone. Clinic B: not taking new patients this month; suggests calling back on the 1st; no reduced fee; walk-in mornings only. Note what the front desk told you.

3 My list of places to try
Using the official finder or advice line on the handout, write down places you could call. You don’t have to do this now.
