Without JavaScript, this is a blank record to print and fill in by hand. Use your browser’s Print command. Write one session per row and add up the columns at the end. Count people, never names.

Languages usedThe languages the session was taught or interpreted in, for example: English, Spanish. Accessibility requests metHow many requests you met, for example large print, a hearing loop or captions. Count the requests, not who made them. Feedback sheets returnedHow many of the anonymous feedback sheets came back.

Your numbers

Counted by you, kept by you.

These numbers are yours. Moraph for Good does not collect them: this page sends nothing anywhere. Use them however your organization reports its programs: a monthly count, an annual return, a note to a funder or your board.

Count heads, not people’s details. There is nothing to sign and no list of names. If someone prefers not to be counted, leave them out.
